Attach a written summary of the situation and any documentation (such as listing agreements, purchase agreements, buyer contracts or MLS datasheets) which support your request. |

Requests must be filed within 180 days of the transaction or when the facts constituting the matter complained of could have been known in the exercise of reasonable diligence. |

Include 4 copies of the request and the attachments. |

Include a check payable to MNAR for $250 (Each party is required to submit a $250 deposit. The prevailing parties' deposit will be returned and the non-prevailing parties deposit will be retained by MNAR to offset the administrative costs related to the proceeding.) |
